Introduction

To assist local governments in establishing vocational rehabilitation service desks for persons with disabilities (hereafter referred to as “vocational rehabilitation desks”), the program aims to manage employment transition notifications and services for individuals with disabilities. By integrating various service resources, vocational rehabilitation case managers provide personalized services based on individual needs, abilities, and interests. These efforts effectively connect and utilize local vocational rehabilitation resources to ensure continuous, seamless, and appropriate professional services throughout the rehabilitation process, thereby facilitating employment for persons with disabilities.
 

Legal Basis

This service is provided in accordance with Article 33 of the People with Disabilities Rights Protection Act.
 

Service Description

1. Following the vocational rehabilitation service process for persons with disabilities (see attached table), professional services are provided to help individuals achieve suitable employment.
2. Vocational rehabilitation case managers adopt a career development perspective to establish a collaborative relationship with clients. Based on individual needs, they conduct assessments and jointly discuss with clients to develop vocational rehabilitation service plans. These plans include pre-employment preparation, job matching, employment support, and post-employment stabilization support services.
3. Case managers also connect resources from vocational rehabilitation project organizations, employment centers, or other relevant units to offer services such as career counseling and consultation, vocational assessments, vocational training, workplace internships, workplace learning and readaptation, supported employment services, sheltered employment services, home-based employment, entrepreneurial guidance, job redesign, and other related programs that facilitate employment for persons with disabilities.

Target Audience

Vocational rehabilitation case management services are primarily aimed at individuals with disabilities who require extensive or continuous vocational rehabilitation support to achieve employment or career goals.
